2025-2026 - First Reconciliation and First Eucharist Registration

Thank you for partnering with us to prepare your child(ren) to receive the Sacraments of First Reconciliation and First Eucharist. Your child(ren) must be enrolled in either Faith Formation or attend a Parish School in order to complete this form. WE CANNOT PROCESS YOUR DATE REQUEST UNTIL THE BAPTISMAL CERTIFICATE IS RECEIVED. If your child was NOT baptized at Annunciation Catholic Church, you must provide us with a legible copy of their Baptismal Certificate. Please upload a copy of the Baptismal Certificate, email to vridgeway@annunciationorlando.org or drop off a copy to the Parish Office. If your child's Baptismal Certificate has been prepared in another language, we would kindly ask that the Baptismal Certificate be translated prior to submitting. Thank you!
