Welcome to Annunciation Catholic Church Faith Formation Registration.

You are about to complete registration for the 2012-2013 Faith Formation program. This registration includes our Sunday Nursery, Preschool, Elementary and Middle School sessions. Please complete both the family information form (page 1) and the student(s) information form (page 2) in their entirety to ensure proper registration.

The cost to register one student is $80, two students is $100 and three or more is $150. This is for the year and includes all materials they will need for class. After you have submitted the form, you will be presented with payment options including scholarship or delayed payment due to hardship.

Please know that we will never turn anyone away due to financial difficulty.

Our Sunday school classes are full. Please send an email to be placed on our waiting list and we will contact you as soon as there is an opening. Thank you.

Starting on Wednesday, September 19 please note that you can still register for Faith Formation classes, however you will be notified as to what class your child is in and your child will not be able to begin classes until the second week.
